Set up a regular donation

Please enter all forenames as they appear on your Grand Lodge Certificate.
Please enter your surname as it appears on your Grand Lodge Certificate.
It is vital that you enter your Grand Lodge Reference Number correctly so that your donation may be correctly matched to your name. Your Grand Lodge Reference Number may be obtained on Portal, or by asking your Lodge Secretary.
Begin typing your Lodge name, then ensure you select the correct Lodge from the list. It is vital that you select the correct Lodge, otherwise your donation will not be credited correctly.

I understand that donations to the Centenary Hall Stone Appeal are not eligible for Gift Aid and should not be declared on any tax return as a charitable donation. I also confirm that either:

(a) the details given above are my own personal details and the donation will be made from my own personal funds

OR

(b) I am completing the form on behalf of another UGLE Freemason who has authorised me to do so and the donation will be made from that individual’s personal funds.

Data protection notice

The data you provide on this form will be used to:
1. Notify you when you have qualified to purchase one of the Centenary Building Jewels, and send you details of how to make the purchase.
2. Share your name, Grand Lodge Reference Number and donation amount with your Metropolitan, Provincial or District area, as well as the Lodge to which you have chosen to assign your donation, for the purpose of enabling them to monitor their aggregate donations and to recognise individual contributions from their members.
3. Maintain a record of your donation for the purpose of assessing your current or future eligibility to purchase each type of Centenary Building Jewel during the Centenary Hall Stone Appeal and any extension of that appeal.
4. Maintain a permanent record of your eligibility to wear any Centenary Building Jewel, as well as your Lodge’s/Province’s/District’s/Metropolitan area’s eligibility for recognition as part of the Appeal, and to share that record with any relevant Masonic Authority.
