I confirm that I am the Metropolitan, Provincial, or District Grand Treasurer, and that either:

(a) this donation is solely made up of surplus amounts which have arisen as part of the Metropolitan, Provincial, or District area’s normal activities (e.g. membership fees) and not from any other income source,

OR

(b) where any part of the money has arisen from returns on or disposals of investments (including any form of rent and property income), all amounts due to HMRC (or the relevant tax authority in the Metropolitan, Provincial, or District area’s jurisdiction) have been declared and paid in full, without any deduction taken for this donation. I also confirm that I am aware of the current HMRC (or relevant tax authority) position relating to taxation of investment income by clubs.
